What This Does

Before you roll a program out widely you pilot it, and the pilot needs a feedback form. Help me create a form in Google Forms drafts one from a description, so you can spend your time on the questions that matter to the decision.

Before You Start

  • Google's support page says this feature requires an eligible Google Workspace or Google AI plan, is rolling out gradually and may not be available to you yet, and is currently available on desktop. If you do not see it, ask your Workspace admin, or build the form by hand.
  • You know the decision the pilot feeds (extend, change or stop) and what you need to learn.
  • Your company's survey practice allows Google Forms for employee feedback.

Steps

1. Open a new form

On your computer, open a new form in Google Forms. Google's page says a window opens where you can enter a prompt. You can also click Help me create a form in the toolbar.

2. Describe the form

Include details. Google's page says richer prompts give better results. Try:

Prompt

Create a feedback form for participants in a four-week pilot of a new-manager program. Ask how relevant the content was to their day-to-day work, whether the time commitment was workable, how confident they feel about applying what they learned, what to change, and one open comment. Keep it under ten questions.

Click Create.

3. Review the draft

Read every question. Remove leading questions. Check that rating scales are consistent. Add a question about whether the participant's manager gave them time, if that matters to your decision. Google's page says you can edit your prompt at the top of the draft and click Submit, or click Recreate for a new version, and you cannot go back to the previous version after that.

4. Know the limits

The page says the feature does not support multiple sections yet and does not edit an existing form. It replaces it with a newly generated one. Generate first. Edit by hand afterward.

5. Create the form

When you are happy, click Create form. Send yourself the link and complete it once as a participant.

Real Example

Scenario: You are piloting a coaching skills workshop with two cohorts of first-time supervisors and need feedback within a few days of the last session.

What you type: The prompt above, adjusted for a workshop.

What you get: A draft with rating questions and two open-text questions. You cut one duplicated question, reword "Was the program useful?" to ask about a specific task, and test it on a colleague.

Tips

  • Decide what you will do with each answer. If a question cannot change a decision, delete it.
  • Anonymous or attributed? Choose deliberately and tell participants which one it is.
  • Google's page includes a separate feature for summarizing responses. It is an option for later.

A Note on Your Data

A feedback form collects opinions about a manager or a program from named or identifiable employees. Decide whether the form will be anonymous before you send it, and do not put learner names or performance ratings into the prompt. Small pilot groups can be identified even without names, so report results in combined form.
